Senior Options & Solutions (SOS)
According to the most current data available on the Town of Southold website, 24.1% of Southold Town’s population is made up of retirees. That’s approximately 6,092 individuals across the entire Town. As costs of living rise across North and South Forks of our island, younger individuals seek a less expensive lifestyle elsewhere, causing the median age of our population to rise. SBELIH is committed to strengthening our community by improving quality of life for older adults.
To meet the ever-evolving needs of our community’s aging residents, Stony Brook Eastern Long Island Hospital’s Geriatric Center of Excellence developed the Senior Options & Solutions Program.
Headed by Juliet Frodella, CRC, LMHC, CCM, Senior Options and Solutions (SOS) is a free program and provides assistance and resources to older adults, families and caregivers who need help answering healthcare questions, identifying their needs, and accessing necessary services. Juliet is not only the Director of SOS, but she is also the Director of Community Care and Practice Administration.
A care manager is available to assess each individual’s needs at a convenient location― in the home, at community centers, or at the SOS office, now located across the street from the Hospital at 222 Manor Place. After a review of topics such as medical status, mental health status, housing and in-home needs and social functioning, the care manager connects the individual with services to encourage independent living and optimize good health.
After assessment, it is very common for your loved one to be referred to alternate community services that specialize in particular needs. Our care manager’s expertise and knowledge of available options benefits seniors through professional referrals and direction. Some of these services include social interaction/programming, cognitive programming, nutrition, benefits assistance, home services, and housing/placement.
