The Case Manager is responsible for performing integrated concurrent review and provides all aspects of Discharge planning for an identified group of patients. Is responsible for the planning, directing, implementing, coordinating and improving the discharge planning process. Oversees the development of coordinated, integrated and individualized discharge plans including social work intervention if appropriate.
Mandatory three years combined clinical and administrative experience in an acute hospital setting. Experience in Utilization Management, Performance Improvement and D/C Planning preferred.
Registered Professional Nurse currently licensed in the State of New York, BSN - Case Management Certification, PRI - Screening Certification, BLS. AAS From accredited school of nursing.
Good vision and hearing. Maintains physical and mental capabilities to perform work related duties.
